Applying for a UDID card — a step-by-step guide

Everything families need to navigate the Unique Disability ID application confidently.

The Unique Disability ID (UDID) is a single, nationally-recognised card that gives you access to schemes, concessions, and rights under the RPWD Act. The process is simpler than it looks, but a few details can slow you down.

Documents to gather before you start

  • Recent passport-size photograph (JPEG, under 100 KB)
  • Colour scan of a signature (JPEG)
  • Aadhaar card and one address proof
  • Any earlier disability certificate, if issued
  • Recent medical reports relevant to the diagnosis

The five stages

  • Register on swavlambancard.gov.in with an OTP-verified mobile number
  • Complete the personal, disability, and employment sections
  • Upload documents and submit for CMO verification
  • Attend the medical board — bring originals and one attendant
  • Download your UDID once approved (typically 6–16 weeks)
