Applying for a UDID card — a step-by-step guide
Everything families need to navigate the Unique Disability ID application confidently.

The Unique Disability ID (UDID) is a single, nationally-recognised card that gives you access to schemes, concessions, and rights under the RPWD Act. The process is simpler than it looks, but a few details can slow you down.
Documents to gather before you start
- Recent passport-size photograph (JPEG, under 100 KB)
- Colour scan of a signature (JPEG)
- Aadhaar card and one address proof
- Any earlier disability certificate, if issued
- Recent medical reports relevant to the diagnosis
The five stages
- Register on swavlambancard.gov.in with an OTP-verified mobile number
- Complete the personal, disability, and employment sections
- Upload documents and submit for CMO verification
- Attend the medical board — bring originals and one attendant
- Download your UDID once approved (typically 6–16 weeks)
